In the Season 6 premiere, Sarah and George's wonderful news signals the beginning of a new era at Ash Park. Sarah becomes involved in the lives of the Blighs, a wealthy Australian family. She develops an immediate connection with handsome and charming widower George, as well as his modern young daughter Anna and withdrawn daughter-in-law Olivia. The final scene is of Sarah's death at the age of 104, surrounded by David, Olivia, and Leah. On her nightstand is a copy of a book entitled A Place to Call Home, written by a Samantha Swanson, Andrew and Olivia's granddaughter.
Elizabeth sets up her maternity hospital, and the family urges Carolyn to reach out to Jack. Upon returning home from Israel, Sarah and George host a family gathering to announce a surprise, but an unexpected reunion between Henry and James causes tension. Meanwhile, Anna hides a secret from the Blighs while staying in Hawaii with Olivia and Matthew. The 1950s come to an end at Ash Park, bringing with it love, forgiveness and acceptance for the Bligh family. Dawn's funeral brings healing for some, while the consequences of Henry's accident brings soul searching for others. Elizabeth questions her place in the family.

As the new year dawns, Harry and Henry profess their love for each other, while Jack finally accepts Carolyn's olive branch. Doris and Sarah break the news to Dawn's daughter Emma that her mother has died. Roy moves into Ash Park after finding it difficult to return home.
During the lead-up to the new year, a number of significant changes occur. George, Sarah, and David prepare for Israel. Jack decides to go to Burma to reflect on his wartime service. Carolyn suddenly returns to Inverness to confront Jack and asks to go with him to Burma. Elizabeth confides in Sarah that she has been diagnosed with a terminal illness. Delia leaves Prudence as all the interior design work has been completed in the Swanson home.

It was later announced, on 19 March 2018 that the 10-episode sixth season would be the series' last. Four months after Prudence’s party, Carolyn and Jack are living separately, and one of Anna’s novels has been adapted into a radio serial.
When top surgeon Neil Burton arrives at Inverness Hospital, it becomes immediately obvious that he is not there for Henry's job. Jack and Carolyn are shown travelling to Burma. Carolyn later becomes Inverness's first female MP. Jack received an Order of Australia for his services to rural medicine.

On Christmas Day, George and Sarah host a party for many of their friends and family, including Elizabeth, Roy, Doris, Jack, and a visiting Frank Gibbs. At Prudence's home, a smaller contingent gathers, including Carolyn, Anna, Olivia, Andrew, Delia, Henry, and Harry. All of the Bligh's family and friends are happy with the new directions their lives are taking, with the exception of Jack and Carolyn, who are both miserable.
Production on the sixth season began on 19 March 2018. Anna arrives back in Sydney with some exciting news from Hollywood. Matt returns to visit Elizabeth and give her the keys to Douglas's house. Four months after the party at Prudence's, Jack has still not spoken with Carolyn. As the Blighs celebrate Christmas and Hanukkah, Jack slips further into depression, until a conversation with Frank inspires him to make a courageous decision.
